WEBNov 15, 2022 · INTRODUCTION. The fractional excretion of sodium (FENa) measures the percent of filtered sodium that is excreted in the urine. This calculation is widely used to help differentiate prerenal disease (decreased kidney perfusion) from acute tubular necrosis (ATN) as the cause of acute kidney injury (AKI, formerly called acute renal failure).

WEBIn this study, FENa (>1%) had a specificity of just 54% in detecting intrinsic kidney injury. However, it is important to note that an FENa of <1% may still provide value in patients on diuretics, as noted by the high negative predictive value of 82% in this study (table 3 in the work by Abdelhafez et al. [ 4 ]).

WEBNotes. In this calculator, mcg is the abbreviation for micrograms. The FENa helps differentiate acute tubular necrosis from a prerenal state. FENa >2 % usually indicates ATN. FENa <1% usually indicates a prerenal state such as dehydration or other forms of prerenal azotemia. FENa's between 1% and 2% may be seen with either ATN or prerenal state.
